Without continuity a team is rarely in a position to isolate the causes. All GMs think they provide the talent. All coaches think they coach well enough. In the first year or two of a new regime they can say that and people give them slack. But if they've been there for 7 years and the losing happens, they are better able to run a good assessment. Stable processes, stable systems, archive of player evaluations, film base to see improvement or lack thereof..etc.
Football coaches and GMs sometimes have too much ego.
